Services You Can Pre-Register For
Currently, you can pre-register for the following departments and procedures at Simi Valley Hospital:
- Nancy Reagan Breast Center (mammogram)
- Radiology (X-ray, CT or MRI)
- Laboratory
- Aspen Surgery Center
- Hospital inpatient and outpatient surgery
- Scheduled outpatient procedures (GI Lab, peripheral vascular)

What You Will Need to Pre-Register
Before you begin the pre-registration process, you will need:
- Your insurance ID card information
- Procedure date and time
- Type of procedure (such as a mammogram or X-ray)
- Personal contact information
- Physician order.

After You Have Finished Online Pre-Registration
After you have submitted your online pre-registration form, you will receive a confirmation e-mail from the hospital. You will also be reminded to print your completed form and bring it with you on the day of your procedure, along with any items listed on your pre-registration form.
